MRI AUTHORIZATIONS
Most insurance companies require prior authorization for MRIs. If your insurance company does not require authorization, you may go ahead and schedule your appointment.

If authorization is required, our office must obtail prior approval for you before you schedule your MRI appointment. If you had a previous MRI, our office will try to obtain authorization for the facility where you last had your MRI. If you plan to go to another facility, you must call the authorization department at 631-462-2225 ext. 238.

If this is your first MRI, we will pick a facility. If you plan to go to a specific facility, you must call the authorization department at 631-462-2225 ext. 238. You may leave a voicemail with your name and the name of the facility you'd like to go to.

Vytra Health Plans: (Vytra patients must go to the facility on the insurance card.)

• Please do not schedule your appointment prior to our office contacting you with your authorization number. As soon as our office obtains the authorization, we will contact you with your approval number. This may take 7 - 10 business days, depending on your insurance carrier. Workers' Compensation cases can take 30 days.

PHYSICAL THERAPY AUTHORIZATIONS
The following insurance carriers require a prescription from our office, as well as a referral from the primary care physician for physical therapy.

• Aetna HMO/AUSHC [Requires prescription along with a primary care physician referral.]
• Aetna [Requires prescription along with a primary care physician referral.]
• Vytra HMO [Requires prescription and a primary care physician referral.]
• Oxford Health Plan [Requires authorization for physical therapy. You must contact our office with the name of the physical therapist you will be going to. We cannot obtain authorization without this information. Please call 631-462-2225, ext 238 with the facility name so that we can promptly obtain your Physical Therapy authorization.]
