WELCOME NEW PATIENTS

Dear Patient:

Welcome. At Long Island Spine Specialists a team of health care professionals will see you; a welcoming receptionist, a nurse, and a licensed Physician Assistant (PA) who works along with our Board certified Spine Surgeons and Board certified Pain Management Specialists. The medical team, PA and Physician, work together to develop an effective treatment plan for you. (For more information about Physician Assistants, please read the Physician Assistant FAQ)

On this page, you'll find a full packet of forms you'll need to download and complete prior to your visit, as well as important information regarding your first visit. Please be sure to read and complete all documents. NOTE: Forms can be downloaded at the bottom of this page.

LISS is located on the 2nd floor. After exiting the elevator, you will make a left in our suite to the reception desk. At this time, all your forms will be collected and you will be registered. We request that you arrive 15 minutes prior to your appointment on your initial visit so all necessary paperwork can be completed and you are kept on schedule.

If you are unable to keep your appointment, please notify us at 631-462-2225 so we may accommodate another patient.

IMPORTANT NEW PATIENT INFORMATION
Please bring the following when you come for your first visit:

• All prior medical information including x-rays, CAT scan films, MRIs and the accompanying medical reports . We do need to see the actual films.
• All forms (found below) must be completed prior to your visit. Please complete them at home or this may delay your being seen at your appointed time. This may result in a possible need to reschedule. If you are unable to complete the forms, please bring them 30 minutes prior to your scheduled appointment and we will assist you.
• You must bring your health insurance card with you. We will need to scan the card into our computer system.
• If your insurance requires a referral from your primary care physician, please call that physician for the referral number or bring your paper referral. If you do not have this number or paper referral, you will be responsible for the bill at the time of your visit.
• If your visit is due to an accident, such as a Workers' Compensation or No Fault claim, it is imperative that we have all insurance information prior to your appointment. If you did not have this information when your appointment was made, please call our office prior to your visit so that we can verify the information with the insurance company. If you do not provide this information, you will be responsible for payment at the time of your visit.
• All fees and co-payments are due at the time of treatment. Please refer to your insurance plan or contract for information on what charges are covered. Any charges that are not covered are the patient's responsibility and are payable at the time of service. Payments are acceptable in cash or by check or credit card.
