AUTHORIZATIONS

WORKERS' COMPENSATION AUTHORIZATION
All Workers' Compensation authorization requests can take up to 30 days before they are approved. This includes MRIs, CAT Scans, Physical Therapy, EMGs, Injections and Surgery. We will mail you a copy of your authorization once it is received. You are responsible for scheduling MRIs, CAT Scans and physical therapy appointments once authorization has been received.

• EMG Tests: [Our office will contact you to schedule your EMG appointment when we receive authorization from your Workers' Compensation carrier.]
• Injections: [Once we receive authorization for your procedure, a confirmation letter will be sent to you along with a copy of your authorization. Upon receipt of that letter, you must call 631-462-2225 ext 244 to schedule a procedure date.]
• Surgery:[Once we receive your surgery authorization, a confirmation letter will be sent to you along with a copy of your authorization. Your surgery will not be scheduled until your next appointment. Please call the office and make an appointment to follow up with your surgeon ]
• MRIs [One Call Medical or Med Focus may contact you to schedule your MRI since some compensation insurances use their own vendors to schedule MRIs. Due to quality concerns, please contact our office so that we may direct you to a preferred facility to schedule your MRI. Pursuant to Workers' Compensation Law, the treating physician and injured worker have the right to select the diagnostic facility of choice.
