Office hours are Monday-Thursday 8:30am -4:30pm, Fridays, 8:30-am-4pm. For urgent medical needs after hours follow the prompts for our answering service when calling the office. For non-urgent medical needs such as appointments, prescription refills, or test results, call during normal office hours or utilize My Chart
appointments
Call 631-837-2359 for an appointment. Please arrive timely and call in the event of lateness. Although some delays are unavoidable, missing an appointment time may result in rescheduling due to scheduling constraints. Please give at least 24 hours notice for a cancellation.
Appointments can be rescheduled and cancelled through My Chart.

In the event of a weather related closure, LISS will do it’s best to contact our patients prior to the storm to reschedule appointments. In the event of an unplanned closure, you will receive a text message notification. If you don’t receive a text, call the office. The outgoing message will be updated with closure status. INITIAL CONSULTATION
Please make sure to bring your insurance card(s) in addition to any Workers Compensation case or No Fault carrier/accident information and any referrals required by your insurance plan to your first visit. You may complete your pre-registration before your appointment through Catholic Health MyChart. If you have previously had an MRI or CT scan related to your condition, please bring a copy of the imaging disc to your appointment.
PRESCRIPTION REFILLS
Please be advised that refills require 48 hours’ notice and can only be generated during business hours (no weekends.) Pain medications can not be prescribed if you have not had an office visit within the past 90 days. Refill requests can be made by contacting us by 631-837-2359 or via a message in the Catholic Health My Chart. If your refill has been approved, you will be notified that it has been prescribed to your pharmacy.
DISABILITY FORMS
Due to the extensive nature of many forms and the volume of requests received daily, please allow 3-7 business days for completion. A fee of $15-$250 may apply depending on the length and complexity of the form.
REFERRAL POLICY
CONFIDENTIALITY: MEDICAL RECORDS
All medical records are confidential. If you need medical records, they can be obtained in your Catholic Health My Chart account or by calling the office at 631-837-2359. The medical records department requires at least five business days to process requests and there is a fee associated with copying records. Long Island Spine Specialists maintains compliance with HIPAA regulations regarding confidentiality and has an Identity Theft Compliance Plan.
